Please submit your completed application along with all required documents to the Office of Admissions. You are encouraged to contact H.M.I. directly to determine whether additional admission materials are required beyond those listed below. To ensure prompt processing of your application, please follow these instructions carefully.<\/p>\r\n\r\n

  • Two Passport size photos (with your printed name and signature on the back)<\/li>\r\n \t
  • $75.00 non-refundable application fee (made payable to Hope Medical Institute)<\/li>\r\n \t
  • Two\u00a0original or notarized<\/strong>\u00a0copies of your birth certificate\r\n(Original birth certificates may be obtained from the Department of Health or Department of Vital Records in the state you were born in)<\/li>\r\n \t
  • Two\u00a0notarized<\/strong>\u00a0copies of your passport page that shows date of birth<\/li>\r\n \t
  • Essay \u2013 \u201cWhy I Want To Become A Physician\u201d<\/li>\r\n \t
  • Two\u00a0official<\/strong>\u00a0high school transcripts, sent directly from the institution<\/li>\r\n \t
  • Two\u00a0notarized<\/strong>\u00a0copies of high school diploma<\/li>\r\n \t
  • Two\u00a0official<\/strong>\u00a0transcripts from each university\/college attended, sent directly from the institution<\/li>\r\n \t
  • Two\u00a0notarized<\/strong>\u00a0copies of all degrees you have received<\/li>\r\n \t
  • Official<\/strong>\u00a0or\u00a0notarized<\/strong>\u00a0SAT or ACT scores (if the exam was taken)<\/li>\r\n \t
  • Official<\/strong>\u00a0or\u00a0notarized<\/strong>\u00a0MCAT scores (if the exam was taken)<\/li>\r\n \t
  • Two\u00a0original<\/strong>, signed academic letters of recommendation<\/li>\r\n<\/ul>\r\n

    All applicants can submit unofficial\/student copies of transcripts for processing only.<\/strong><\/p>\r\n\r\n

    However, if admission is granted, official copies of transcripts will be required and should be sent to Hope Medical Institute directly from the institution. Transcripts downloaded from the Internet will not be accepted. Letters of recommendation can be sent via e-mail by the author for processing only.<\/p>\r\n\r\n

    Please note that college graduates who have already received degrees must also provide a notarized copy of their high school diploma and transcripts or can have official copies sent directly from the institution.<\/p>\r\n\r\n

    In addition, please make sure that your application form is filled out completely and that you have attached a current passport size photo (with your printed name and signature on the back)<\/b> with a paperclip.
